| Pharmacological effects | 1. Calcium dobesilate has the effect of inhibiting vasoactive substances (histamine, serotonin, bradykinin, hyaluronidase, prostaglandin) on microvessels. The high permeability of the blood vessel reduces the damage of the vascular intima, improves the biosynthesis of basement membrane collagen, improves the permeability of the capillary wall, and improves the flexibility of red blood cells, the blood viscosity is reduced, and the lymphatic reflux is improved, thereby reducing the pathological high permeability of the microvascular wall. 2. Calcium dobesilate can reduce the levels of fibrinogen and globulin, reduce the high polymerization of red blood cells, activate the activity of fibrinase, increase the dissolving ability of blood fibrin, thereby reducing blood viscosity. Reduce the synthesis and release of platelet aggregation factor, inhibit a variety of aggregation factors, cause aggregation reaction and platelet spontaneous aggregation, reduce platelet activity, prevent thrombosis, its effect is dose-related. 3. For microvascular disease, calcium dobesilate can increase the caliber of arterioles and veins, increase the number of capillary openings, significantly accelerate blood flow, and significantly improve microcirculation. | adverse reactions | calcium dobesilate has very little chance of side effects. the main adverse reactions are fever (26%), gastrointestinal discomfort (12.5%), skin reaction (8.2%), arthralgia (4.3%) and granulocytopenia (4.3%). no death has been reported. |
| pharmacokinetics | after administration of 500mg calcium dobesilate for oral administration, the blood drug concentration level during the 3rd and 10th hours was above 6 μg/ml, and after 6 hours (t maximum) the blood drug concentration (c maximum) reached the maximum, with an average of 8 μg/ml. The blood concentration at 24h after medication was about 3 μg/ml. The protein binding rate is 20% ~ 25%. Trace amounts are present in breast milk. Calcium dobesilate will not enter the enterohepatic circulation, it is mainly excreted in its original form, and only 10% is excreted as metabolites. Within 24 hours after administration, about an oral dose of 50% is excreted in urine and about 50% in feces. The plasma half-life is about 5h. |
